Please print out and review our office financial policies and  fill out completely the patient history form ( both pages) prior to your arrival in our office.  Be sure to bring all vision and medical insurance information with you at the time of your exam.  If we are unable to verify your coverage, you will be responsible for all charges until verification can be obtained and a refund will then be issued.
