Payment of all copays is expected
at the time of your appointment. Please bring all insurance information with you, including insurance cards,
to your appointment. If your insurance is not listed above, payment of all professional fees is due at the time services
are rendered. As a service to you we will gladly submit your insurance claim for you to your insurance company.
